Answer: C. Aller forms the passé composé with être, and with être the past participle agrees with the subject in gender and number. The subject elle is feminine singular, so the participle takes an -e: est allée. B uses the right auxiliary but leaves the participle unagreed, which would suit a masculine subject. A and D use avoir, which aller does not take in this construction, and D compounds the error by adding an agreement avoir would not trigger here anyway.
